The directory includes Primary Care Providers, Hospitals, Pharmacies, and other providers, with maps and directions. For questions or concerns regarding the Online Provider Directory, please feel free to contact us. To begin your search, select a Provider Network and enter a City and State or a Zip Code and Radius. Optionally, you may also narrow your search by entering additional search criteria in step 3.
If you are an existing UnitedHealthcare Services Company of the River Valley, Inc. member and would like to search the provider network that you have access to, please refer to the network listed on the front of your ID card and then select that network in the dropdown box below.
Many of you have access to the UnitedHealthcare national network when traveling outside of the UnitedHealthcare Services Company of the River Valley, Inc. service area. For example, if you have UnitedHealthcare Options PPO or UnitedHealthcare Choice Plus on your ID card, then you have access and can search the UnitedHealthcare national network for hospitals, providers, and other health care professionals. PLEASE NOTE: While every provider listed contracts with us to provide medical services to our members, some providers listed may not be accepting new patients at this time. For the most current information regarding the status of any provider's practice, please contact the selected provider or our Customer Service office nearest you. The online provider directory is updated daily. Information about a provider is provided to us by the provider and is verified as part of the credentialing process.

The listed medical providers are independent contractors, not employees or agents of the health plan. Members and their medical providers, not the health plan, decide what medical care members receive and how they receive it. UnitedHealthcare only determines what medical care will be paid for under the applicable benefit plan.
